Coming to the Office to Renew Your License / Certification
We do not accept cash. Acceptable forms of payment are as follows:

accepted. You will be required to have a valid photo ID or drivers
license when renewing in person.
Nursing Assistant Certification division accepts Visa or MasterCard

The Board is asking for your cooperation in facilitating processing of
renewal and other licensure applications. We are using forms designed
to be scanned into an imaging system to process paper applications.
These applications can not be reproduced correctly by your average
printer or photocopier. The
following licensure applications are NOT available for download.
